Sample Thrive Assessment

Thank you for your interest in the Thrive Assessment for Children's Homes and Orphanages!

This sample assessment includes 31 questions to introduce you to the Thrive Assessment tool. To complete the assessment, please read each question and rate your home on according to the following scale:

• 0 means “Does not exist or never happens”
• 1 means “Applies to less than half of children or circumstances”
• 2 means “Applies to half of the children or circumstances”
• 3 means “There are a few reasonable exceptions” and
• 4 means “Fully functional on a consistent basis or 100% of children”

If a question is not applicable to your home, please skip it. After you finish, your home will receive a score and summary where you can review your answers and consider next steps in order to improve.
